Centroid Kit #: K(4-7 digits)This(K) Kit orserial number is on every Centroid control and is required information.Thisisthe reference numberwe will use to follow all of your repairs, now and in the future. See the picture sets at [Finding your Kit #] these will help you find your Centroid Kit and serial number.
Time Saving Note
Before removing your control, take multiple pictures of all of the wire connections. Label all wires (Blue painters tape) so that you can easily re-install them and so you do not cause an issue by placing a wire or fiber optic line at the wrong location. This will save you time later.
All control repairs: We need your complete control so that we can bench-test the entire assembly and make sure that everything is working before we return it to you.

Repair Policy:
- a) We charge a $400.evaluation fee. Minimum Charge, even if you decide not to repair the unit.
- b) We can provide you with either a verbal or written repair estimate before ordering parts.
Please note: We could find hidden problems that will change this estimate as we work on your repair.
- c) If there are any changes, we will contact you for approval to proceed withyour repair.
- d) We cannot stock all older PC and control parts. We order replacement parts, receive then, install them andthen bench test your control for 6 hoursor more. Then werebuild your system, and do the final testing, invoice your repair and schedule the return shipment.
Standard Payment Terms:
- a) Payment by Check only. Payment in full, before your control is returnshipped to you.
We are no longer using Credit Cards (because of the costs) or wire transfers.
Packaging directions and requirements:
- a) Pack it – Secure & Professionally – Insure the shipping to us for Minimum …. $ 2,500.00
If your control is lost it cannot be replaced. This means that you will have to purchase a new system from
Centroid.$2500.will not cover your loss, this does provide you a higher level of monitoring your package.
- b) Cover the LCD Monitor glass with cardboard or foam board. (Use Blue painters tape to hold it in place)
- c) Bubble pack or Paper-fill the box completely. No Packing Peanuts, EVER!(See why Below)
- d) Ship the control without the keyboard, as we need the entire control to properly test it.

(M400 Atrump 2019 to now – Only the all-in-one PC/LCD combination is needed – see shipping pictures below)
e) Using the RMA form – list all of the problems you are having. We may need to ask specific detailed questions about the errors you are having during the repair.
Packaging notes:
1) Your RMA number needs to be on your package or processing delays could be incurred.
2) Proper packing “No peanuts” ever – or delays and additional costs will be incurred.
POLICY – We will not open packages at our Service Lab., when peanuts are found.
Packages opened and if peanutsare found, are then taken to our warehouse,opened, peanuts removed,
controlcleaned, static energy grounded and then the control is returnedto our labso we can start your evaluation.Expect a 1-2day delay and added travel and labor charges.
Why NO Peanuts: 1) The Mess 2) Possible Static electricity damage to your control.
3) The peanut (floating particles) could negatively affectother control repairs in process and/or could cause instrumentation errors at our service center.
Our established procedures and policies:
- a) We will not service any control that has a past due invoice to: Us, Centroid or any Dealer.
- b) YourCompany Name &RMA Numberis how we track andaccessyour service activity&history.
- c) We always email you a note when your kit arrives at our facility.
- d) We are the largest Centroid board level technical repair service center in the USA.
- e) We have multiple units arriving weekly;we are a high volume service center, our policies have been established to serve you better and enables us toprocess repairsefficiently.
- f) We service Centroid and many other PC based CNC Controls.
- g) If we do not get everything we requested from you at the start, your service will bedelayed!
- h) If you poorly package and ship your control to us. We will repackage it properly,before we return shipping it to you, we will charge you for the additional packaging, labor and materials.
Hand delivery to us by Local Customers:
We allow customers to hand deliver and pick up controls to our facility to save shipping costs.
These are always by appointment only, as we are not on site every day. You should not arrive expectantly as we are not always on site.
We recommend that you use UPS shipping.
We havehad Fed-Ex damage multiple shipments. These were all properlypacked.
This is why we no longer use, or recommendthat you use them.
Packing Hints:
We are providing some shipping and boxing suggestions. The way you pack your control for shipping to Centroid Repairs is very important, as we do not want to have any shipping damage.
Of course, use your experienceto assure proper packing of your control.
Always take pictures of tear down and packaging.
Make sure that you insure (High value $2,500.) your control shipment for both loss and/or damage.
We can not stress enough that these items are very important !
M15 Packing also M10 & M20
The first Centroid M10, M15, M20 control units are all a single black box style PC control with a 10” B&W or color LCD. Packing is the same for all of these models.M10,M15 & M20 Unit measurements are:
17 wide x 14 tall x 10” deep. Home Depo sells a box that is: 21 w x 15 t x 16” deep and this is goodoff the shelf box providing a good amount of packing. Make sure to cover your LCD as they are very expensive to replace.
M40-M400 PC Style Packing
All of these Centroid Models have a PC mounted in the cabinet. You can purchase a heavy duty box from Lowes/Home Depo/U-Haul. Ours was from Lowes 24 x 18 x 18”. Then we cut it in half on the 18” side to make it: 24 x 18 x 12” as shown below. Then taped the 2 half’s backtogether double walling the ends in the process.
M400 1995-2009 all 10-14" LCD
These are a PC unit with LCD mounted on a swing arm. Again purchase a heavy duty box from Lowes/Home Depo
or U-Haul. Ours was from Lowes 24 x 18 x 18”. Then we cut it in half on the 18” side to make it: 24 x 18 x 9” as
shown below. Then taped the 2 half’s back together double walling the ends in the process. Always cover the
LCD for addedprotection.
M400 2010 - Present
These are a single PC unit with a surface mounted LCD. Again purchase a heavy duty box from Lowes/Home Depo or U-Haul. Ours was from Lowes 24 x 18 x 18”. Then we cut it in half on the 18” side to make it: 24 x 18 x 9” as shown. Then taped the 2 half’s back together double walling the ends in the process. Cover the LCD for added protection and build up packing to assure safe shipping as our pictures show and recommend.
T400 Lathe
The lathe controls are the same as the mill controls (M=Mill & T=Turn/Lathe) Many lathes have separated MPG handwheels – we do not them. Remove the PC based control unit, the power supply with cables and ship these to us.Review PC packing from the M400 2010+ to present as they are similar and some T model pictures are located there.
Shipping loss and Insurance:
We cannot and will not be responsible in any way for in bound shipping damage or loss. There is very little help we can offer if your control is lost by your shipping agent.
Additionally, if it arrives damaged – your insurance coverage must cover the additional repairs!
If external packaging damaged is seen at arrival, we will inform you ASAP/the same day it arrives, we will take pictures – it is your responsibility to contact the shipper and collect for this damage.
YOU will have to do the work to get compensated by your shipper. If damaged – we will provide you with pictures, details and a brief written statementso that you can make your claim to your shipper. Should you want us to do any additional claim work, we charge for any additional claim work at shop rate of $150.00 p/hr.
